Schedule your sports physical at the Nelson County Health System Clinic, in McVille or Michigan. The cost for the physical is $30 and they do not accept credit cards (cash or check only). Please call the McVille Clinic (701)322-4347 or the Michigan Clinic (701)259-2118. Reminder, the McVille Clinic is open Monday through Friday 8am to 4:30pm and the Michigan Clinic is open Tuesday and Thursday from 9am to 3pm. If you are planning to use your preventive benefits, such as well child visits or preventive physical in your health insurance plan, you must let the clinics know when you make the appointment. Please have the required physical forms completed and signed prior to your physical. Physical forms can be found on the Dakota Prairie Website under the tab “forms” and then click on the link entitled “physical form”. Or on the Lakota Website by hovering over “more” tab, then hovering over on the “sports/activities” and clicking on the link entitled “Sports Physical Form”.
